QUESTIONNAIRE

  • Questionnaire is a proforma, which is to be filled out by a respondent furnishing factual information. It may have similar physical appearance as an interview schedule; but it is filled in by the respondent himself. Whereas, interview schedules are filled in by the investigator, who asks questions to the respondent and records the answers.
  • The questionnaire is generally sent through the mail by the respondents to be answered as specified in a covering letter. The covering letter should
    • Explain the aims and objectives of the study.
    • Have notes regarding operational definitions.
    • Ensure confidentiality.
    • Contain promise to send a copy of the final report (if possible).
    • Details about answering the questions.
