URETHROTOMY

Indications

  • Urethral obstruction

Anesthesia control - In large animals

  • Epidural anaesthesia
  • Controlled in standing or dorsal recumbent state.

Site

  • In dog-at the seat of obstruction (The commonest seat is behind the os-penis.

  • In cattle
    • Sigmoid flexure : About 3 inches behind the scrotum along the median line.
    • Ischeal site: From 2 inches below the ischeal arch downwards along the medial line

Technique

In dog

  • Catheter is passed through the urethral opening to the level of obstruction.
  • Incise on the obstruction site.
  • The calculi are picked up and removed

  • Caetheter is passed into the bladder through the normal opening to ensure patency of passage.

  • The catheter is retained for a few days.

In cattle

  • Post scrotal method
    • A midline incision 3 inches long is made about 3 inches behind the scrotum.
    • Retractor penis muscles reveal by dissecting areolar tissue or either side.
    • Body of penis is exposed.
    • Make an incision on the urethra along the exact midline.
    • Obstruction  is relieved and catheter is inserted for its patency
    • Thin plastic tube may be left insitu  for one or 2 days.
    • The wound is left open to heal by second intension.
  • Ischeal urethrotomy
    • Two inches skin incision is made along the midline starting from about 2 inches below the ischial arch downwards
    • This exposes two retractor penis muscle.
    • Body of penis exposed, Palpate and incise the urethera along the exact midline
    • Obstruction  is relieved and thin plastic catheter may be left insitu for one or 2 days.
    • The wound is left open to heal by second intension.
