Preparation of case records and reports regarding children's problems and treatment in a child guidance clinic is very necessa­ry. The reports should include:

1. History of the physical and mental growth of the child and the family
2. Description of his environment - home, school and neighbourhood
3. Symptoms
4. Examination and interview
5. Diagnosis and prognosis
6. Various stages of treatment and progress
7. The nature of the follow-up in each case
8. Rehabilitation
